Accurately reporting accidents and injuries play a major role in all safety management systems. It is crucial to implement and establish an injury reporting procedure that includes employee communication and participation across all departments.  Encouraging each employee to report injuries or near misses improves workplace safety and the organizational safety culture.

To that end, in today's post we will discuss some best practices related to an effective injury reporting system.
  • All injuries should be reported to the nearest manager or safety officer. Subsequently, the Manager should determine the next cause of action and treatment options.

  • When completing the injury report, it is important to accurately describe the injury events in chronological order without downplaying or embellishing the severity of the injury or the events leading to the injury.

  • Information gathered from individuals who do not have firsthand knowledge or information relating to the injury cannot be used in reporting.  The injury report should not contain second or third hand information.
 
  • If you are not a trained medical professional, refrain from using diagnostic terms when filling out the injury report. Diagnostic terminology such as sprains, torn ligaments and broken bones should be avoided until after the injured party receives proper medical attention. Descriptions such as “severe pain in lower left leg, individual cannot put pressure onto leg and cannot walk without assistance” can be used in place of “potential broken left leg”. 
 
  • Take note of details such as weather conditions, time of day, shift period, sublocation, department and specific hazards in the immediate area.
  • Ensure that any first aid or medical intervention that was provided to the injured party is properly documented. 
 
  • Deploy a digital injury reporting application. Managers and safety personnel can file reports using a cell phone or tablet to improve data collection and response times.

Remember, injury reporting is an effective tool for minimizing future injures. In addition, it positively influences safety culture and increases employee communication and participation.

​The American Petroleum Institute (API) on July 14 issued the second edition of its landmark API Specification Q2, Quality Management for Service Supply Organizations for the Petroleum and Natural Gas Industry. ​The new edition strengthens safety, quality, environmental protection and sustainability requirements to facilitate the implementation of best-in-class standards across natural gas and oil supply chains.  There will be a six-month window for organizations that were previously Q2 certified to update their current quality management system to meet compliance with the new edition. 
The 2nd edition contains several significant changes, including:
  • Alignment with API Q1 verbiage where applicable
  • Removal of the reference to the outdated version of ISO 9000
  • Addition of requirements a) through e) to Human Resources section (4.3.2)
  • Addition of supply chain to Purchasing Control (5.6.1)
  • Alignment of Control of Testing, Measuring, Monitoring, and Detection Equipment with API Q1 (5.8)
  • Addition of Control of TMMDE to list of exclusions
  • Removal of Preventive Actions section.

Employees of should be trained on safety the safety management system. It is of no importance, for example, to have fire extinguishers on the rig if employees don't know how to use them.

Technically, the contingency plan is a document that identifies the actions the organizations will take when a specified incident happens.  The goal is to minimize the incident’s impact to your operations, employees, supply chain and customers. To that end, we have detailed 8 steps for effective contingency planning:

  2. Record the business processes for each department.
  3. Identify issues (real and potential) that can negatively impact business operations.
  4. Review the issues list with department managers and rank contingency priority to help focus plan development.
  5. Create plans to address each prioritized issue.  Try to minimize the number of steps required to resume operations.
  6. Test your contingency plans. If a portion of the plan is flawed or conflicts with other contingency plans, modify and then reassess the plan.
  7. Formalize and communicate the plan with applicable personnel within the organization.
  8. Finally, make sure to review the plan at pre-determined intervals to insure continued relevance.   This review will help identify areas for improvement.

By following the steps outlined above, you will be well on your way to creating effective contingency plans that chart realistic tactics and achievable goals in addressing potential operational disruptions.

SDSs are designed to be user-friendly and must communicate the properties of each chemical product, including: the physical, health, and environmental hazards; protective measures; and safety precautions for handling, storing and transporting the chemical.

Employers are required to ensure that SDSs are readily accessible to employees for all hazardous chemicals in their workplace.
